Simply print the two pictures below (right-click - view image - save image as - print), and - on the day of your appointment - ask your Affinity Estate Planning Consultant to sign it. Then send it back to us - and, as soon as we have received it, we will send you your M&S voucher, just for seeing us!

TERMS & CONDITIONS

To claim your £10 M&S voucher, you need to return a claim form with your name, reference no or postcode and Affinity’s Estate Planning Consultant signature on it.


Claim forms without Affinity’s Estate Planning Consultant’s signature will be deemed invalid.


It is your responsibility to ensure Affinity’s Estate Planning Consultant signed the £10 M&S claim form on the day of his visit.


It is your responsibility to ensure that signed £10 M&S claim form is returned to Affinity within 30 days of the visit.


Affinity cannot be responsible for lost and or destroyed vouchers.


Only one voucher per family can be claimed. Consecutive visits (follow-ups) do not constitute a valid claim for a £10 M&S voucher.


All £10 vouchers will be sent within 28 days of the receipt if the claim form.

REFERRALS


Offer applies to referred clients where Affinity completes an instruction under our standard pricing and terms. To qualify for a £100 referral reward, a person you introduce to Affinity must make a purchase of a minimum sum of £500. If a person you introduce to Affinity makes a purchase of up to £499, a standard referral reward of £50 will be issued and they will receive a £50 discount on the services. A referral fee will not apply in cases where two for one or other discounted products / services are provided. The offer is valid from 15th August 2012 and expires on 30th November 2012.
